Charmed Again:
After Phoebe has her premonition about Paige at Prue's funeral, demonic bounty hunters show up to "eliminate" Belthazor. We see the funeral hall empty where seconds before Phoebe's premonition, there was a line of people waiting to give their condolences to the girls, Victor, Leo and Cole.
Piper does not light the candles before she summons Grams, but in the episode 'PreWitched' (when Grams summons Patty) we see that the candles must be kept alight to maintain the connection with the dead.
Charmed Again (2):
Phoebe and Piper seem surprised at the fact that a white-lighter can impregnate someone, because they are dead. Piper says "how does a white-lighter get somebody knocked up?", but when she went to the future she saw herself with Leo's baby, and Leo told her that he hadn't clipped his wings.
When Phoebe and Piper were trying to help Paige use her powers, they said that she should wave her arm at the candle because thats what Prue did. But when Prue got her powers, she controlled her magic with her eyes not her arms.
Hell Hath No Fury:
When Paige wants to cast the "Instant Karma" spell on her coworker Donnie, she decides to "change 'demon' to 'dirtbag.'" But the spell was visible to the audience earlier in the episode, and did not contain the word "demon." While the word was in the instructions for the spell, the spell itself specifies a "villain" as the target.
After the "dirtbag" is brought back to the manor and Piper is ragging on him, Piper gets very angry and decides to blow her husband up. Less than a minute later, Cole says "Back away slowly, Leo." Leo says "What?" and Cole says "She's becoming a fury." Then it shows Pipers hands, which now have long black fingernails, but they just suddenly show themselves right when Cole decides to show up and say she's becoming a Fury. When she blew Leo into orbs there were no long black nails.
Enter the Demon:
After Phoebe and Paige have switched bodies and Poebe is trying to teach Paige how to levitate, Phoebe's hair switches from her shoulder to her back then to her shoulder again.
When Piper and the Zen master switch bodies and Yen Lo uses the dragon blade to steal Piper's soul, the Zen master's image is shown as a visualization of the soul, but it should be Piper's image instead.
Size Matters:
Shortly after Phoebe frees the blonde girl from the clay, she tells her to run "before Gammill sees [them]." But Phoebe hadn't learned the demon's name at any point. Leo and Piper do find out his name, but not until later.
Piper suggests that Paige orb in place, because the clay is still wet enough that it will collapse if she's not there to support it. But when Paige actually does orb, the clay cracks and collapses into powder as if it had been bone dry.
A Knight to Remember:
When Phoebe picks up the cordless phone to call Paige before the Shocker Demon attacks for the first time, the cordless phone is white but then after the attack, when the cordless is on the floor, it's black.
When the prince is supposed to be frozen on Paige's living room couch, when they do a close-up you can see the actor blink.
When Phoebe tried to kick the Shocker Demon, she became electrocuted until Piper interposed with a large wooden stand. But when the prince attacked the Shocker Demon with his metallic sword, the demon was cowed and the prince was unharmed.
According to Paige's "fairy tale" the witch ends up with the prince. If this is really a past life, the sorceress ended up with the prince in the Middle Ages. So why do they have to fix it? Everything was just going the way it did according to her fairy tale, and good magic was obviously still present in the world.
Brain Drain:
Right after Piper comes out of her crazytown adventure, she comes down the stairs in completly different clothes and hairdo than she went in with.
If the vanquishing spell doesn't work in the Halliwell Hospital place, then the Power Relinquishing Spell shouldn't work either.
Black as Cole:
After Cole explains he might not be able to regain control after embracing his demonic half, he tells Phoebe she should have the Belthazor vanquish potion ready just in case. But in "Look Who's Barking", Piper tried using that potion on Cole and it didn't work because he had himself magically protected from it.
In this episode Cole says "upper level demons have a human form and they bleed red, not green", but when Cole told Phoebe he was half human he said "how else could I bleed red, even in my demonic form".
Paige looks in the Book of Shadows and suggests that the demon they are looking for could be Belthazor, not knowing that Baelthazor is Cole's demonic form. But we saw Phoebe making extensive changes to Belthazor's entry in the Book of Shadows in a previous episode, and the changes are not in evidence here.
When Paige cuts the flesh away from Sykes, you can visibly see a white bandage-like thing under his skin as it comes off.
Muse to My Ears:
The one demon that knew about the 40s party was vanquished, and he went straight from spying on Paige and Phoebe at P3 to "glamouring" into Leo at the Manor. He had no way of telling Devlin about the 40s party, and yet it was still full of demons waiting to kill the Charmed Ones, with Devlin "glamoured" into Melody as a trap. And Devlin didn't even have that power - the demon that was vanquished did.
If Leo can't hear the girls call him when HE is underground, he shouldn't be able to hear the girl's call him when THEY are underground and he doesn't know they are at P3 until a few minutes later.
After Phoebe jump kicks a warlock and falls to the ground, there is a blonde woman standing next to them. In the next shot of the warlock, when Piper blows him up, the woman is gone. But all of the mortals had been frozen, so she couldn't have moved.
Why did Phoebe have to write a spell to vanquish the warlock in this episode? When they have vanquished warlocks before they used the power of three spell, "the power of three will set us free".
Near the end of the episode, in the scene at P3, Phoebe launched a jump kick at a warlock after Paige has been knocked down by a small energy ball. Everybody else in the club was frozen by Piper, but when Phoebe lands she jostles a blonde extra who moves from her pose.
A Paige from the Past:
After Piper freezes the first crime scene that Cole and Phoebe (aka Frankie and Lulu) have hit, Piper unfreezes Darryl by touching him but at the beginning of the the episode when Piper and Cole are touching the car that Piper froze, to stop it from exploding after a car crash, while trying to help the woman inside, the car never unfreezes. This is somewhat inconsistent through the entire show a lot of times they can unfreeze things by touching them and then a lot of times they can only unfreeze things by Piper using her pointer finger to either unfreeze a head or a foot or some other body part.
Paige and Leo are in Paige's 1994 room for at least 1 minute and don't smell the cigarette smoke but when Paige's father enters the room, Paige hugs him and in the middle of the hug he immediately smells the cigarette smoke.
On the wall of Paige's bedroom when she was a teenager is a Metallica poster for "Garage Inc.", a CD not released until 1998 although Piper later says that Paige has gone back to 1994.
When we see Paige's 1994 bedroom, she has what appears to be a Bride of Chucky doll. The movie "Bride of Chucky" wasn't released until 1998.
Trial by Magic:
In the teaser, Phoebe has a vision of a murder. When she first sees the shadow of the attack, the assailant is holding his victim down with his right hand while stabbing with his left. Then when the vision focuses on the Medusa head tattoo on his forearm, it is the right arm that is stabbing.
Charmed and Dangerous:
In this episode, the Source talks about the "Ultimate Power" being the Hollow. But in Season 8, we learn that the "Ultimate Power" is Billy and Christy. (And they end up taking in the Hollow, so now are they the "Ultime Power" times 2?) Which is it? It can't be both.
Marry-Go-Round:
When Paige was invisible and Cole watched the blood, there was a large pool of her blood, but in the next shot, the pool of blood was smaller.
Saving Private Leo:
In this episode, an invitation to Piper is addressed to 1329 Prescott Street. But in episode 1-7: "The Fourth Sister," the Halliwell sisters put up a flyer for their lost cat, and it gives their address as 7571 Prescott Street.
When Piper freezes an object it always continues with its path when it unfreezes, but when she unfreezes the knife Rick has thrown at her it drops to the floor.
Leo has always stipulated that he cannot heal himself, but when he gets stabbed by Rick his wound instantly heals itself.
When Rick and Nathan held Leo down so he couldn't get to his charge, why didn't he orb to her side to save her?
The name plate under the photo of Leo at the Veterans Association indicates that he died on November 24, 1942. Paige's research indicates that Nathan and Rick both died on the same day as Leo, but she later finds out that they were buried at the V.A. on November 22, 1942, two days before they are supposed to have died.

In the scene where we first see Phoebe and Cole when she's about to leave the apartment you see she is wearing gold and white high heels but as soon as she reaches the elevator shes wearing black boots actually to be more specific they're Uggs, a type of boot.

In the teaser, when Leo levitates to hang a picture, he has a major wedgie and the ankles of his pants hike up to mid-shin due to the harness that is used for the levitation effect.
