A Witch's Tail (1):
In this episode, when Piper and Leo are preparing the nursery, Leo tells Piper he has been watching her her whole life, and she says "That's a little bit creepy". she acts as if she doesn't already know this but in episode 2-22 'Be Careful What You Witch For' Piper says, "You have been watching me my entire life".
Paige says she cannot wait to tell the local witch doctors that Borneo demons are not impervious to magical powers. But in the episode 'House Call', they act as if they have never heard of witch doctors, let alone dealt with them.
A Witch's Tail (2):
When Phoebe goes to sign her papers at the end, she spells her name as Phoebe.
Happily Ever After:
If all the objects in the fortress are originals from the fairy tales, why are the ruby slippers red? In the original story the slippers were silver, they were made red for the MGM movie, so they would be the brightest thing on the screen.
After Piper puts Little Red Riding Hood's cloak on and travels into the book we see a close up of the book and the cloak is still lying behind it.
When Adam spills coffee all over Phoebe's top in the office, the stain disappears and reappears between shots.
Siren Song:
When the Siren is being healed, you can see that she's just holding the pole next to her, when it's supposed to be going straight through her.
When Piper and Paige fall out the window and land in the dumpster, in the wide shot the garbage bags are white, and in the close-up they're blue.
Witches in Tights:
When Leo and Ramus are talking in front of the bay windows, the door behind Ramus switches from closed in the wide shots to open in the close-ups.
The drawings of the sisters, change from when they were in the book, to when the Aggressor tore the paper.
In the scene where Phoebe dangles the slumlord from the roof, the TransAmerica Pyramid can be seen behind Phoebe and Cole. A subsequent shot, after Phoebe tosses the slumlord back onto the roof, is take from the opposite side, but the TransAmerica Pyramid is still behind them, and is much closer.
The Eyes Have It:
When Paige is telling the others she thinks Lydia is the one Cree was after, she says 'I think Lydia was the one who summoned that dust storm', but none of the sisters were present when the dust storm was summoned. They only arrived afterwards.
When Phoebe had her premonition of Lydia being attacked by Cree, why didn't she immediately warn her? Lydia is a Gypsy high priestess, which Phoebe knows because she just saw Lydia perform a religious ceremony, and she also knows from the previous events in the episode that Gypsy elders are aware of Wiccan magic. But instead she said nothing and waited to talk to her sisters before doing anything about the premonition. So although they were able to save Lydia, she wound up being needlessly attacked and injured.
Lydia has green eyes, but when Orin extracts them at the morgue, the eyes he puts into his head are blue.
The sisters act as though they have never heard of the evil eye before; but in episode 1-2 'I've Got You Under My Skin' the demon Javna invokes the power of the evil eye to steal youth from women.
When Phoebe goes to get her palm read, Madame Theresa draws lines on her hand. But when she first enters the room and sits down the lines are already visible on her hand, before they have been drawn there.
Sympathy for the Demon:
There are many references in this episode to Phoebe using a power-stripping potion on Cole. But it wasn't Phoebe who used the potion on Cole; it was Emma, the fiancée of one of his victims.
Cole says Barbas can use his powers to become the Source, but in the episode "Womb Raider" they established that an aspiring Source needs to lay his hands on the Grimoire to become the source, which was hidden away by Leo in the Andes.
A Witch in Time:
In this episode when Phoebe and Piper have left the car to go and find Miles (to stop him dying), the second time after past Piper has warned present Piper to go right not left, Phoebe's shoes change from what appear to be black boots or high-heels to flat beige knee length boots.
Sam I Am:
In this episode Paige is assigned Sam, her father and Patty Halliwell's former whitelighter, as a charge but in the episode P3 H20 we find out that Sam clipped his wings - "Piper: Sam was mum's whitelighter?" "Leo: Emphasis on was. When your mom died, Sam clipped his wings. He's mortal now, but once, yeah he was a whitelighter. He looked after your mother"
When Phoebe cut Cole in 'A Witch's Tail' the blood was acidic and melted through the letter opener, but in this episode the blade was complete and had drops of blood on it.
When the Charmed Ones decide to try to vanquish Cole, Paige says that the newest potions they mixed should work, since they're "at least as strong as the ones [they] used on the Source." But they never used any potions that had an effect on the Source.
When Paige and Phoebe are sitting at the bar at the beginnning of 'Sam I Am' their arms change positions between every shot.
The Importance of Being Phoebe:
When Phoebe is lying on the ground and is supposed to be unconcsious, when Cole goes to pick her up you can see Phoebe throw her arms around his neck.
When Cole and Kaya are taunting Piper and Paige about the fact that they now own the manor, they hold the deeds out in front of them laughing for a good 5 seconds. Did they, and more importantly did Paige herself, forget that she can orb things to her? She could easily have summoned the deeds and orbed out of the house before Cole could stop her.
When Kaya throws the knife at Phoebe watch in freeze-frame and you can see that for the shot from behind her there's no knife in her hands.
Paige's green top that covers her black t-shirt constantly changes from being on her shoulder to off, particularly in the police station scene.
Centennial Charmed:
In 'Centennial Charmed' Paige orbs to another reality where she loses all her powers. Leo tells her the this is because she was never called as a witch in this world, but Paige had most of her powers before she was called because of her whitelighter blood, so she should retain these. Leo says that Paige has assumed the life of this reality's Paige, but then adds "...or at least the life she would have lived if she hadn't been killed". If alternate Paige had lived she would have had Whitelighter powers, even if she didn't know about them, so our Paige should also have them.
When Paige follows Phoebe into the kitchen and is trying to convince her that she is telling the truth, Phoebe's right earring is missing in the shots showing both women, but it is there when the shot changes to close ups of Phoebe. It is an awfully large earring to be hidden by her hair in the two-shot.
Although the actress has changed, Piper's doctor appears to be the same character as Eva, the shuvani, Paige even says, 'we saved her shuvani tribe'. But throughout this episode they call her Eve.
Paige's headstone reports her year of birth as 1975. But in episode 4-2: "Charmed Again, Part Two," Paige spoke with the nun to whom she was given as an infant on August 2, 1977, and Paige is always described as having been "given up at birth" by Patty and Sam.
House Call:
Leo orbed from the Manor to Hell while it was full daylight. He orbed Jessica straight to the church, there was a brief scene, and then Leo and Paige orbed directly back to the Manor, but by the time they arrived it was completely dark.
When Piper made the house vanish, and she, Paige and Leo ran up to where it was, the background was obviously fake.

In the beginning, Paige is wearing a shirt with the scrunching sleeve on the right and slanting on the left. In following scenes, the scrunching sleeve is on the left and slanting on the right.

In episode 3-9: "Coyote Piper," Piper danced sexily on the bar while the song "Unbelievable" played. She danced only to the one song before her sisters pulled her off the bar. In this episode, Phoebe and Paige physically enter Piper's memory (magically enhanced for accuracy), and a different song plays during Piper's dance, despite it being the exact same video footage.
Phoebe says to Paige that she didn't know that Kit was their familiar, that she was just some cat that kept hanging around on the porch. But they learned that Kit was their familiar in episode 3-17 'Pre-witched', and they also said that Kit is male, unlike the woman in this episode.
Nymphs Just Wanna Have Fun:
When Paige rearranges the kitchen Piper asks what was wrong with the old system that had worked for the last five years. But when Piper became a neat freak in episode 5-13, 'House Call', she alphabetised all the ingredients in the kitchen, which was not five years before.
Necromancing the Stone:
In this episode Wyatt has his wiccaning. After all of the vanquishing is done with the Necromancer, it's finally time for the wiccaning and the spell that is said is this: "I call forth from space and time, Matriarchs from the Halliwell line, Mothers, daughters, sisters, friends Our family spirit without end, To gather now in this sacred place And help bring this child to grace." In this spell it is said to bring forth "daughters and sisters" from the Halliwell line, Prue died in the third season and this is now the fifth season, shouldn't she have been summoned as well to bring her nephew Wyatt to grace?
Piper mentions that Grams banished the Necromancer "sixty years ago" at Patty's Wiccanning. Grams soon thereafter says that the Necromancer has had "almost seventy years" to regain his strength. However, Patty was born in 1950, and this episode aired on 5/4/03, at which time Patty's Wiccanning could not have been more than 53 years earlier.
When Paige casts the truth spell on Nate, Nate just starts telling her the truth about everything like how he's married and has kids BUT when Prue cast the truth spell a question had to be asked in order for the truth to be told.
